The bae from Bay recreated to the best of my ability. There are mods used, but only for textures (hair colour from Cazzonero's New skin and hair colors mod as the original blue is either too bright or too dark; clothes colours from A copious amounts of colours by Konata Inoue, mostly for the purpose of having a truly white top; and eye texture from Eye Texture Project found on steam workshop). I wish there was a more accurate hairstyle in game, but sadly there isn't. Same goes for the ability to have some hair sticking out, should you wear a beanie (yes, I know there's mods, but clipping's a bitch). Enjoy if you also can't let go.

As for the outfit, the top is exclusive to SRIV but the rest is present in the Third. If anybody's interested I can also post the alternative timeline Chloe as well as BtS's one, as I have already found the right outfits for either.
