An easy to use .PSD template. It's up to you to add anything on it, this is just the basic.

YOU MUST DOWNLOAD AND INSTALL THE FONT FIRST :

I know, it's not the most accurate font but, because I wanted to keep the template free (and legal to share)
I used a free font.

So just open the PSD, edit the 2 text layers and you're good!
