I still think time travel may be a good idea.
The other idea i had was Saints challenging gods. And then Gat challenged Satan - it wasn't that far.
I wish that our main character can perform more action (not only running, jumping, swimming - just on surface, taunting and shooting).
And I hope that he will eat, drink and smoke again, like in Sr2. And I want a energy bar (not only stamina and life bar)
I like Emy's idea about the criminal that sells human organs. Very nasty. But why always Shaundi? I think if he kidnapp Matt, it would be better, he's young, healthy and probably he doesn't even do drugs, so he would be a perfect candidate. (with Shaundi, we would end to have again the 'Damsel in distress' clichè, unless Volition would put a new twist in this trope).
The new enemy can also be italian (oh, I read it now you though the same. I would choose male. Italian mafia is very very very old fashioned). Did you noticed that even if SR is a game about gangs, there are no italian gang? It would suit well.
Also, I don't want anymore the Boss changing look during mission (no more 'My name is Cyrus Temple' and 'From Asha with love' like missions - Though I would like infiltrations in other ways)
And, about upgrades, I don't want them forced on MC when you finish missions/mini games. I want to be able to choose and to remove upgrades for MC whenever I want.
